Some employers require a choice of super fund form for payroll. In this case, download and complete the form that matches your account:
Once completed, give the form to your employer or payroll team.
If your employer asks for compliance documentation, download the CFS Letter of Compliance and include it with your form.
Provide your USI and the CFS ABN so your employer can pay contributions into your account.
